wedding guestbook pages?

vernacular_lifeI cant for the life of me remember what the 'real' name of the guestbook pages were that I had at my wedding. They were kind of like forms where you filled out your name, how you knew the couple, favorite memories, predictions, etc.
I thought it was guestbookpages.com, but that's not it.
the company did bridal shower ones and baby shower ones
anyone remember???
